BHS: Youth Risk Behavior Survey

Dear Parent/Guardian,

On March 11, 2015, Bedford High School will again be conducting the Youth Risk Behavior Survey.  This survey is offered to high school students nationwide every two years. This survey is optional and anonymous.   The survey is a vital part of our overall effort to help students avoid risky behavior and make healthy choices.  Nevertheless, we understand if you do not want your child to participate in this survey.  You may opt out by returning the form below by March 9, 2015.

The survey was developed by the Center for Disease Control.  The survey asks students about six types of health-risk behaviors.  These behaviors contribute to the leading causes of death and disability among youth and adults.  These are:

  1. Behaviors that contribute to unintentional injuries and violence
  2. Sexual behaviors that contribute to unintended pregnancy and sexually transmitted diseases, including HIV infection
  3. Alcohol and other drug use
  4. Tobacco use
  5. Unhealthy dietary behaviors
  6. Inadequate physical activity
The survey also measures the prevalence of obesity and asthma among youth and young adults.
Bedford has participated since the high school opened in 2007.   The survey is given using paper and pencil.  It is anonymous; no uniquely identifying information is returned to the school and the actual surveys are destroyed.  We receive general information about the risky behaviors prevalent among our students.  You can read the 2013 report for Bedford High School here.  
The Center for Disease Control does not allow us to post the actual 2015 Survey online. We have posted the 2013 Survey here and the questions that have been added for 2015 here.  Between these two documents, you are able to see every question for the 2015 Survey.  Additionally, parents can come in to the main office at Bedford High School to see an actual paper copy of the 2015 Survey if they wish.
